The Health Optimizer app is designed to help you manage your diabetes and track your progress, all at no extra cost. It's the companion app for Transform Diabetes Care and it's where you can access many of the program's features. Use Health Optimizer to sign up for personalized nutrition coaching and more.
Search for the Health Optimizer app in the app store on your smartphone or tablet, then download.
Follow the onscreen steps to sign up and create an account.
Provide your name (exactly as it appears on your member ID card) date of birth and email address.
Create a 4-digit pin once you have successfully created an account.
